Which do you feel is better, eyelid surgery from the inside or outside of the eyelid?

Lower eyelid blepharoplasty can be performed from both the outside (transcutaneous) or inside (trans conjunctival) of the eyelid. 99.9% of the 3000 cases I have performed as an Ocular Plastic Surgeon in Sterling Hieghts, Michigan have been transconjuctival. The advantage of my technique is: NO stitches; NO skin incision, and NO Scalpel blade. I perform the procedure with the MIXTO laser.
This carbon dioxide (CO2) laser allows me to gently remove the fullness ( fat) from the inside of the eyelid. The beauty of this laser is that it stops bleeding as it removes this tissue so the surgery is faster and my patients have less swelling (than traditional methods) and get back to work faster. Instead of removing skin which can leave a tell tale scar line underneath the lashes, I tighten the skin with the Mixto laser! The Mixto is phemomenal to resurface (refresh) the skin. Smoothing of the skin with the laser is a perfect accompaniment to lower eyelid surgery.

Why are my lower eyelids puffy?

The swelling in the lower eyelids is related to the normal fatty tissues that surround the eyes. Fat loves fluid. An irritation, an allergy, an infection amongst other things can cause swelling (fluid uptake) by the fat around the eyes. As we mature this fatty tissue can move forward causing bagginess to the eyelids. At this point cosmetically we begin to look tired and blepharoplasty (eyelid lift) is sometimes the answer.

Dr B I went elsewhere and had eyelid surgery I now have scars. HELP!

I am sorry to learn of your dissatisfaction after your blepharoplasty. Unfortunately, when the upper eyelid incision runs beyond the eyelid crease, the chance of visibility of this line increases. Many times the cosmetic surgeon treats the fullness past the eyelid crease with eyelid surgery, although it is not caused by the eyelid at all. This fullness is usually related to a drooping eyebrow, and the physician would need to lift the brow at the same time as the eyelid surgery in order to avoid lenghthening the eyelid crease incision.
The other thing that I do to avoid a skin incision is during lower eyelid surgery, I use a CO2 laser. Utilizing this laser, I gently remove the fullness (fat) from the inside of the eyelid. There is NO scalpel, NO skin incision, and NO stitches!
That stated, it appears that your scar line is wider than you would like. Speak with your surgeon as he/ she may be able to revise the scar. This should lessen the width, although it may not make it disappear. I have been successful using the Mixto fractional laser on scars to decrease their width and visibility. Depending upon your skin type a permanent tattoo artist may be able to fill in the scar to match the surrounding skin.
Lastly, there are some terrific makeups that should easily cover this area
